As the new Membership Chairperson for NAIN, I have been asked to design a membership survey to be distributed this fall. In preparing this document, I came across a similar survey that you designed for NAIN back in 2001-02. Some of the questions in that survey are no longer pertinent, but other sections continue to be very useful. May I adapt your survey to complete my own? I would of course give you full attribution.
